Ice dams and the late winter melt
An ice dam does its damage quietly. Heat escaping through the roof melts snow, the water runs to the cold eaves and freezes, and the next melt has nowhere to go but up and under. Once the snowpack starts to go, usually the back end of February into March, everything that was frozen in the roof deck lets go at once. That is the week the ceiling calls come in.
By then the drywall above the stain has been damp for weeks. The mold is on the back of the board and in the insulation above it, well beyond the ring you can see.
Bathroom steam and a cold ceiling
Not every ceiling problem starts on the roof. A bathroom ceiling in a closed-up winter house takes a shower's worth of steam each morning.
